Tired Under-Eyes? Filler Isn't Always the Answer

"I look tired even when I'm not."

It's one of the most common things we hear, and it's usually about the under-eyes. Hollowness, darkness, crepey texture, the shadow that shows up in every photo taken from below.

The reflex is to reach for filler. Sometimes that's right. Often it isn't — and the under-eye is the single least forgiving place on the face to get that call wrong.

Why the under-eye is different

The skin there is the thinnest on your body. There's minimal fat padding, the anatomy is complex, and lymphatic drainage in the area is easily disrupted.

That means hyaluronic acid filler, which attracts water, can look beautiful in the right candidate and puffy or blue-tinged in the wrong one. It also means the problem is frequently not volume loss. It can be skin quality, pigment, allergies, sleep, or a shadow cast by a structure well outside the tear trough.

A good consult sorts out which of those you actually have.

What PRF does instead

PRF under-eye rejuvenation doesn't add volume the way a filler does. It improves the tissue itself.

We draw a small sample of your blood and process it — using the Selphyller™ system, without anticoagulants — into platelet-rich fibrin: a matrix concentrated with your own platelets, stem cells, and growth factors. Injected into the under-eye, it releases those growth factors gradually, stimulating collagen and improving skin quality over time.

Depending on what your skin needs most, we may use PRF, Selphyller gel, or PDGF. That's a clinical judgment made at your appointment, not a preset package.

It's a good option for people who want a fully natural approach, since everything comes from your own body and there's no foreign substance to react to. All injectables carry some risk, though, which is why candidacy gets confirmed at consultation.

The timeline is the important part

This is not an instant-result treatment, and you should know that going in.

  • Most people notice improvement around four to six weeks

  • Optimal results land around two to three months

  • We recommend a series of three treatments within six months

The gradual pace is the mechanism, not a limitation. Your own biology is doing the work, so the change reads as you looked rested, not you had something done.

Pricing is $700 per session. If you want more structural volume — temples, midface, marionette lines — Selphyl® Biofiller uses your PRF to create a natural filler-like gel, priced at $1,000.

Downtime

Mild swelling, bruising, or redness at the injection sites, typically resolving within a few days. It behaves like other injectable treatments. Review the pre- and post-care instructions before you come in — what you do in the week beforehand affects your bruising more than anything we do in the room.

Don't schedule this within two to three weeks of an event.

When we'd suggest something else

Sometimes the honest answer to under-eye concerns is medical-grade skincare and better sleep. Sometimes it's neurotoxin elsewhere on the face. Sometimes it's hyaluronic acid filler placed nowhere near the tear trough. And sometimes it's a referral to an oculoplastic surgeon, because what's bothering you is a fat pad that no injectable is going to fix.

We'd rather tell you that than treat you.
