Botox or Filler? They Solve Completely Different Problems

This is the most common confusion we see, and it's not the patient's fault — the two get lumped together constantly as "injectables," as if they were interchangeable.

They aren't. They do close to opposite things.

The short version

Neurotoxin relaxes muscles. Botox, Dysport, Jeuveau, Xeomin. It temporarily reduces the muscle contraction that creases the skin above it. It treats movement.

Filler adds volume. Hyaluronic acid gel placed to restore or enhance structure. It treats loss and shape.

Movement versus volume. Once you have that, most of the confusion resolves.

The test that sorts it

Look in a mirror with your face completely relaxed.

Lines that disappear at rest — the forehead line that only shows when you raise your brows, the crow's feet that appear when you smile, the "11s" between your brows when you concentrate. Those are dynamic lines. That's neurotoxin.

Lines and hollows that are still there when you're not moving — the crease from nose to mouth, flattened cheeks, thinning lips, temple hollowing, a softening jawline. Those are structural. That's filler, or a biostimulator.

Where it gets complicated

Plenty of concerns are both. A deep line between the brows may have started as movement and etched itself in over years — relaxing the muscle stops it deepening, but the existing crease may need something in it too.

And some things that look like a filler problem aren't. A downturned mouth corner is often muscle. Under-eye darkness is frequently skin quality rather than volume, which is why we sometimes recommend PRF instead.

This is the actual job of a consultation: figuring out which of these you have before anything gets opened.

Neurotoxin isn't reversible, but it's temporary, which makes it low-stakes. Filler lasts longer but can be dissolved. Different kinds of safety net.

Which to start with

If you're new to injectables and torn, we usually suggest starting with neurotoxin. It's temporary, the result is predictable, and it's a low-commitment way to find out how you feel about having had something done.

There's no rule against doing both in one visit, and many patients do. But there's also no rush.

The answer that's sometimes neither

Regularly, what someone describes as needing filler is actually skin quality — texture, laxity, tone. That's microneedling, peels, and medical-grade skincare, not a syringe.
