MPWR’d Membership Terms and Conditions

Enrollment Agreement

By enrolling in the MPWR Aesthetics and Wellness (“MPWR”) Membership Program (the “Program”), I agree to the following terms and conditions:

1. Monthly Auto-Debit Agreement

I authorize MPWR Aesthetics and Wellness to charge my credit card on file for a monthly savings contribution. I understand that my account will be debited each month for the agreed amount dependent on the level of membership I elect.

2. Non-Refundable, Non-Transferable

I understand that the money I contribute to the Program is non-refundable and cannot be redeemed for cash. The funds will be applied to my future services or product purchases at MPWR only.
Benefits: Any complimentary services or retail credits may not be “stacked” or saved for more than six months past the initial eligibility date.

3. Gift Certificate Option

All banked funds are stored in my Vagaro profile for future use toward MPWR products or services. Should I cancel my membership, I may request a gift certificate for my remaining balance, which can be transferred to another individual.

4. Recurring Payment Agreement

I agree to the recurring monthly payment for the duration of my participation in the Program. Payments will be auto-debited on the 15th of each month, and I am responsible for ensuring sufficient funds. Payments will continue until I notify MPWR of my intent to cancel in writing or via email (hello@mpwraesthetics.com).

5. Failed Payments and Cancellation

If two consecutive payments fail, MPWR may cancel my membership. I will receive notification of any failed payment; thereafter, my account will no longer be eligible for savings or benefits. MPWR is not responsible for overdraft, bank, or late fees. Any discounts already applied will be credited to my remaining balance, or I may be invoiced if the balance is zero.

6. Initiation Fee

I agree to the one-time initiation fee associated with my selected membership level, which is non-refundable, not banked for future use, and covers administrative costs.

7. Terms & Conditions Acknowledgement

I acknowledge that I have read and agree to the full Terms & Conditions for all membership levels, including policies on refunds, cancellations, and payment processing.

8. Termination

To terminate this Agreement, I must submit written notice at least five (5) days before the 10th of any month (the next auto-debit date). Cancellation requests may be sent via email (hello@mpwraesthetics.com) or voicemail (724.201.9424) and must include my full name, email address, and clear intent to cancel. Requests submitted between the 11th and 14th may not prevent the next month’s charge.

Patient Consent & Authorization

By proceeding, I consent to participate in the MPWR d’Memberships under the terms outlined above and accept responsibility for all obligations.

Agreement Terms

  1. This authorization remains in effect until I cancel in writing or via another method agreed upon by MPWR Aesthetics and Wellness PLLC.

  2. I may cancel this recurring payment at any time by contacting MPWR at 724.201.9424 or hello@mpwraesthetics.com by the 10th day of the month.

  3. I understand this Agreement may change; MPWR will provide at least 30 days’ notice of any billing changes.

  4. I certify that I am the authorized cardholder for the credit card on file.

  5. Funds are non-refundable and may not be redeemed for cash.

  6. Funds may be used toward services, products, and treatments at any MPWR location.

  7. Funds may be disbursed as a gift certificate upon request.

  8. Funds will be deducted on the 15th of each month; I am responsible for keeping my payment method current in Vagaro. MPWR is not liable for overdraft or non-payment fees.
